Evidence-Based Management of Hypertension, Diabetes & Chronic Conditions

Chronic medical conditions such as hypertension (high blood pressure), Type 1 and Type 2 diabetes mellitus, hyperlipidaemia (high cholesterol), asthma, chronic obstructive pulmonary disease (COPD), hypothyroidism, and gout require consistent, proactive partnership between doctor and patient. Left unmonitored, elevated blood pressure and blood glucose silently damage blood vessels, kidneys, eyesight, and cardiovascular health long before noticeable symptoms appear.

At Terranova Medics in Trichardt, Dr C. Mbowane provides structured monthly and quarterly chronic disease reviews tailored to South African clinical guidelines. Rather than simply re-issuing scripts, each chronic visit tracks your longitudinal vital sign trends, evaluates medication tolerability, and empowers you with practical nutritional and lifestyle adjustments suited to your household routine.

  • Hypertension (high blood pressure) titration and cardiovascular risk reduction
  • Diabetes mellitus (Type 1 and Type 2) glucometer checks, HbA1c pathology, and foot/eye screening
  • Asthma, COPD, thyroid disorders, epilepsy, and hypercholesterolaemia management
  • Integrated multi-morbidity care for mature patients and pensioners

What Happens During Your Monthly Chronic Check-Up

During a routine chronic care consultation at our Windmill Centre rooms, we record resting blood pressure readings, point-of-care capillary blood glucose, weight, body mass index (BMI), and peripheral circulation checks. For diabetic patients, we schedule periodic HbA1c, fasting lipogram, and renal function (urea, creatinine, eGFR, and urine microalbumin) blood tests to ensure your treatment regimen is protecting your organs effectively.

When adjustments to your medication dosage or drug class are needed, Dr Mbowane explains how each tablet or insulin regimen works, how to avoid side effects or hypoglycaemia, and when to return for your next check-up.

  • Point-of-care blood pressure and blood glucose measurement at every visit
  • Scheduled pathology panels (HbA1c, kidney function, liver enzymes, cholesterol)
  • Medication adherence review and side-effect troubleshooting

Medical Aid PMB / CDL Authorisations & Affordable Monthly Care Plans

Under the South African Medical Schemes Act, 27 Chronic Disease List (CDL) conditions qualify as Prescribed Minimum Benefits (PMBs). Our practice assists medical aid members on Discovery Health, GEMS, Bonitas, Sasolmed, Momentum, Medihelp, and other schemes by completing chronic application forms, supplying accurate ICD-10 codes, and motivating treatment plan updates so your chronic medicine is funded from your chronic benefit rather than exhausting your day-to-day medical savings.

For cash patients and pensioners, our Wednesday Pensioners Day (R300 consultation) and our Silver Plan (R750/month including unlimited GP visits, monthly basic lab test, and chronic care monitoring) make ongoing chronic disease care predictable and affordable.

  • Completion of medical scheme Chronic Disease List (CDL) application forms
  • 6-month repeat chronic prescription management and pharmacy coordination
  • Silver In-House Subscription Plan (R750/pm) includes chronic care monitoring

When to Book an Urgent Chronic Review

Even if your next scheduled script renewal is still a few months away, you should book an earlier review if your home blood pressure readings consistently exceed 140/90 mmHg, your fasting blood glucose readings remain elevated or drop too low, or you experience new symptoms such as dizziness, blurred vision, chest tightness, ankle swelling, numbness in the feet, or excessive thirst.

Early clinical intervention prevents avoidable hospital admissions and keeps you healthy, active, and independent.

  • Persistently high home blood pressure or fluctuating blood sugar readings
  • New side effects such as persistent dry cough, ankle swelling, or dizziness
  • Expired 6-month repeat prescriptions requiring clinical re-assessment

Medical aids, cash & monthly care plans welcome.

R700 Adult Consultation (incl. acute medication) · R600 Kids & Pensioners · R300 Pensioners Wednesday · Monthly Plans from R550/pm.

FAQs

Patients often ask

Can you register my high blood pressure or diabetes on my medical aid's chronic benefit?

Yes. During your consultation, Dr Mbowane completes your medical scheme's chronic medication application form with the required clinical measurements and ICD-10 diagnostic codes so your monthly medication is covered under your scheme's Chronic Disease List (CDL) benefits.

How often do I need a consultation to renew my chronic prescription?

In South Africa, chronic prescriptions are valid for up to 6 months. We recommend a formal prescription renewal consultation every 6 months, with brief monthly or quarterly blood pressure and glucose check-ups in between to ensure optimal control.

What if I do not have medical aid for my chronic condition?

Private patients can consult at our standard cash rates (R700 adults, R600 pensioners, or R300 on Pensioners Wednesdays), or join our in-house Silver Plan (R750/month) which includes unlimited GP visits, a monthly basic lab test, and chronic care monitoring.
