3. Procedure for Requesting Access to Medical Records (PAIA Form 2)
Patients requesting copies of their own clinical records or a medical report for continuity of care may submit a written request with proof of identity (South African ID) to reception at info@terranovamedics.co.za. Formal third-party or legal access requests under PAIA must be submitted on the prescribed PAIA Form 2 (Request for Access to Record) addressed to the Information Officer, accompanied by the statutory request fee where applicable and written patient consent or valid legal mandate.
In terms of Section 61 of PAIA, where disclosure of a health record to a requester may cause serious harm to the physical or mental health or wellbeing of the patient, the Information Officer may arrange for the record to be disclosed alongside counselling by an appropriate health practitioner.
